*제출서류: 

(1) 장학금신청양식 1 부 

(2) 사진 1 매(얼굴확인)

(3) 성적증명서 1부 (대학교 신입생 경우 고등학교 이전 학기 성적증명서)
(4) 자기소개서 1 부(신앙적 배경과 신앙고백, 장래희망 및 계획, 특기 및 수상경력 등을 담은 2p 분량) 

(5) 수업료(등록금)납입 영수증 사본 1 부(신입생의 경우 입학확인서 1 부 추가제출) 

(6) 담당교수 또는 교사 추천서 1 통
(7) 사회봉사 및 교회봉사 증명서류 1 부

유의사항:

이전에 이 장학금을 수혜한 적이 있는 신청자는 다시 신청하실 수 없습니다.

제출서류가 누락되었거나 미비한 경우, 장학금 심사 대상에서 제외됩니다.

2025 Scholarship Application

Please download and complete the scholarship application form below, 

then submit it by Sunday, June 29, 2025, 

either to the DMC Lougheed Office or via email at jeja12@hotmail.com.

Required Documents:

1. One completed Scholarship Application Form

2. One photo (clearly showing your face)

3. One official transcript

   • For university freshmen, submit your final high school transcript

4. One personal statement (2 pages)

   • Include your faith background and confession, future hopes and plans, skills, and any awards or honors

5. One proof of tuition payment (receipt)

   • For new students, please also submit a proof of admission

6. One recommendation letter from a professor or teacher

7. One certificate of volunteer work (including church service and community service)

Note: 

Applicants who received this scholarship in previous years are not eligible to apply again.

Applications with incomplete or missing documents will also not be considered for the scholarship.
